Therapeutic Keratopigmentation — Medically Indicated Cases

What it includes: A comprehensive evaluation to determine whether your condition qualifies for therapeutic keratopigmentation — corneal leucoma, iris defects, post-traumatic disfigurement, LPI-related dysphotopsia, aniridia, or post-surgical opacity. If indicated, Dr. Anchal Gupta performs SAK or femtosecond-assisted KTP using third-generation micronised mineral pigments.

Not for healthy eyes: We do not perform cosmetic KTP for healthy, seeing eyes seeking eye colour change — this is our ethical position and we hold it firmly. If you have a disfigured eye that qualifies for therapeutic KTP, we can help. If you have healthy eyes and want a colour change, coloured contact lenses are our recommendation.

What Determines Your Eye Colour — and Why It Is Hard to Change

Understanding the biology of eye colour helps explain why safe, permanent eye colour change is genuinely difficult — and why the shortcuts marketed on social media carry the risks they do.

Where Eye Colour Comes From

The iris: The iris is a ring of muscle and connective tissue surrounding the pupil. It controls how much light enters the eye by adjusting pupil size. Its colour is determined by the concentration and distribution of melanin, a brown-black pigment, in the anterior stroma of the iris.

Brown eyes: High melanin concentration in the anterior iris stroma. This is the most common eye colour globally, including in most Indians, South Asians, and Africans.

Blue / grey eyes: Low melanin in the anterior iris stroma. The blue colour is not due to blue pigment — it is caused by Rayleigh scattering of light through low-pigment stroma, the same phenomenon that makes the sky blue.

Green / hazel eyes: Intermediate melanin levels combined with lipofuscin, a yellow-brown pigment. Only a small percentage of the global population has naturally green eyes.

Why it matters for colour change: Changing brown eyes to blue requires either removing or covering the melanin. Removing melanin with laser treatment permanently destroys cells and releases particles that threaten the eye's drainage structures. Covering melanin with contact lenses or keratopigmentation is safer because it does not destroy or permanently alter the eye's internal anatomy.

Why 'Eye Colour Change Drops' Don't Work

Various commercial eye drops claim to change or lighten eye colour through melanin modification or other mechanisms. None of these have proven efficacy in peer-reviewed clinical trials. Prostaglandin analogs such as latanoprost and bimatoprost, which are legitimate glaucoma medicines, can cause iris darkening as a side effect — but this is unpredictable, irreversible, and not a cosmetic option. Commercial drops claiming to lighten eye colour are not regulated as medications, are not proven effective, and some may contain ingredients that can cause irritation or harm.

Frequently Asked Questions About Eye Colour Change

Can I permanently change my eye colour safely?

The only safe and reversible option for changing eye colour is coloured contact lenses — which are fully approved by the FDA, AAO, and DCGI India. Among surgical options, cosmetic keratopigmentation (KTP) has the best published evidence, but it is irreversible, carries real risks for healthy eyes including corneal ectasia, and is not approved by any regulatory body for purely cosmetic use. Laser iris depigmentation carries glaucoma risk. Cosmetic iris implants are considered malpractice by leading ophthalmologists. Our honest recommendation for any healthy eye: start with coloured contact lenses.

Are coloured contact lenses safe?

Yes — when properly prescribed and fitted by an ophthalmologist or optometrist. The key word is prescribed. Contact lenses — including coloured zero-power lenses — are medical devices that require a proper prescription to ensure the correct base curve, diameter, and oxygen permeability for your specific cornea. Ill-fitting lenses can cause corneal abrasions, hypoxia, ulcers, and infections. Never purchase coloured lenses without a prescription, from beauty shops, or online without a valid prescription.

What is cosmetic keratopigmentation and is it safe for healthy eyes?

Cosmetic KTP involves depositing mineral pigments into the corneal stroma to cover the natural iris colour. The 2025 PMC review (Springer Ophthalmology and Therapy) considers it the best currently available surgical option for cosmetic eye colour change. However, it is irreversible, off-label (not approved for cosmetic use by any regulatory authority), and has documented risks in healthy eyes including corneal ectasia, photophobia, and dry eye. The AAO issued a warning against cosmetic eye colour change procedures including KTP in early 2024. At Netram, we do not perform it for healthy eyes — we recommend coloured contact lenses instead.

Are iris implants safe for cosmetic eye colour change?

No. Cosmetic iris implants are not FDA-approved, not CE-marked, and are explicitly described as malpractice in a 2022 peer-reviewed systematic review (Alio et al., PMC). Documented complications include glaucoma, cataracts, corneal endothelial cell loss, uveitis, and permanent vision loss. Several patients have been left nearly blind. The devices continue to be marketed in some countries despite this evidence. We strongly advise against cosmetic iris implants under any circumstances.

What is laser eye colour change and is it dangerous?

Laser iris depigmentation uses a Nd:YAG laser to destroy melanin in the iris — lightening brown eyes to hazel or blue. The procedure releases melanin particles into the eye, which can clog the trabecular meshwork and cause pigmentary glaucoma with permanent vision loss. It has no regulatory approval anywhere, no long-term safety data, and is not endorsed by any major ophthalmology organisation. The AAO issued a specific warning against laser iris depigmentation in 2015, noting it is 'untested for safety risks'. We do not perform or recommend this procedure.

I have a disfigured or white eye — can Netram help me?

What coloured contact lenses do you recommend for Indian skin tones?

For Indian skin tones, coloured lenses that appear most natural typically include warm hazel, honey brown, and dark green shades — colours that complement warm melanin-rich complexions. For a more dramatic look, grey, blue-grey, or light honey can create a striking contrast. Can I get coloured contacts if I have a prescription (glasses)?

What happens if I have complications after getting cosmetic eye colour change surgery elsewhere?

Please come to us immediately. Whether you have had keratopigmentation, laser treatment, or an iris implant at another centre and are experiencing complications — pain, redness, blurry vision, raised IOP, or worsening vision — we will evaluate you urgently. For iris implant complications, we can assess whether explantation is needed and refer you to the appropriate vitreoretinal or glaucoma specialist. For KTP-related issues, we can assess corneal integrity, manage infection or inflammation, and plan treatment. Do not delay seeking care if you have post-procedure concerns.
